Please list the title and date(s) of the event you attended and wish to receive a Certificate of Attendance.* This form must also be accompanied by a completed Online Store order for the certificate. Before issuing the certificate, your attendance will need to be verified either with a sign in sheet from the organizer at the event site of a conference, workshop, course, or group webinar, or the registration and login for a individual webinar. If you are an A&WMA member, you may enter multiple events. Nonmembers will need to purchase a separate certificate for each event.
